Cardiac Solutions has a unique approach to guide the care of patients with heart disease. There are several protocol-driven and nurse-directed clinics that provide educational information for patients to assist with the management of their heart health. National treatment guidelines are strictly followed to achieve the highest patient outcomes in all our Clinics and Programs.

This program is designed to educate and manage cholesterol levels. Lowering cholesterol levels is important for all regardless of age, sex, race, or previous heart disease. Having an abnormal level of fat in the blood increases our risk for a heart attack or stroke.

There are two types of cholesterol: “good” and “bad.” It’s important to understand the difference, and to know the levels of “good” and “bad” cholesterol in your blood. Too much of one type – or not enough of another – can put you at risk for coronary heart disease, heart attack or stroke.

Cholesterol Management Program provides:

  • Cholesterol blood test is done with a simple finger stick with immediate results
  • Appropriate medical therapy is prescribed or adjusted
  • Education on various types of cholesterol and cholesterol medication
  • Education on dietary and exercise guidelines

This program is designed to manage your health by learning about heart failure and how to recognize early signs and symptoms of worsening heart failure. Having heart failure may be difficult at times; however, we believe one of the best ways for you to lead a satisfying life is to take part in managing your heart failure. Heart Failure Program provides:

  • One on one session with a registered nurse
  • Onsite blood testing with immediate results
  • Education on what is heart failure, dietary and fluid restrictions, medications and living with heart failure
  • Medication management

This program is designed to educate and manage anticoagulants. Anticoagulants are prescribed to patients with specific medical conditions that put them at risk for developing blood clots. An anticoagulant helps reduce clots from forming in the blood which can cause strokes, heart attacks, or other serious conditions such as blood clots in the legs or lungs. Warfarin, Jantoven, and Coumadin are all examples of anticoagulants. Anticoagulants are different from other kinds of medicines. The effect of anticoagulants on your clotting system can vary from week to week even if you take the same amount of medicine. You will need your blood tested regularly while taking this medication. Anticoagulation Management Program provides:

  • One on one session with a licensed practical nurse
  • INR test which is done by a simple finger stick with immediate results
  • Appropriate medical therapy is prescribed or adjusted
  • Education on dietary recommendation and interactions with medication

Patients learn about alternative anticoagulants and how to recognize any potential problems, about drug to drug interactions while nurses monitor for side effects and patient compliance.

Amiodarone/Multaq/Tikosyn/Sotalol can be very effective in treating your heart condition. Unfortunately, some unwanted side effects from the medication may occur as a result. We will monitor you in a unique way to make sure you are not experiencing any of these unwanted side effects. Arrhythmia Management Program provides:

  • Onsite blood testing with immediate results
  • Heart rhythm monitoring
  • Scheduling recommended 6 month required testing
  • Education on drug-to-drug interactions and medication side effects
  • One on one session with a licensed practical nurse

Cardiac Solutions uses Board Certified Pharmacists progressively in our practice to partner with providers and nurses to optimize health care.

Pharmacy Clinic:

  • Meet one-on-one with a clinical pharmacist to review all your prescription, over-the-counter, and herbal supplements.
  • Your medications will be reviewed for drug/drug interactions, appropriateness of therapy, and side effects.
  • You will be provided a comprehensive medication schedule that will tell you what each medication is being used for and how to take each medication to optimize your medication therapy.  
  • You will have an opportunity to ask questions about potential side effects or any other medication related concerns.

Smoking Cessation Clinic:

  • One-on-one session with a clinical pharmacist.
  • Provides counseling and support via phone or in person for up to 6 months.
  • The pharmacist will use motivational interviewing to develop a patient specific plan to help you become tobacco free.
  • We will help you utilize approved over-the-counter and/or prescription medications to aid in your quit attempt.

We offer regularly scheduled clinics with a device company representative to check your pacemaker or defibrillator on a regular scheduled basis, as directed by your physician. In addition to your scheduled device checks, your physician now has the capability of monitoring your defibrillator or pacemaker from home with a remote communicator. The patient management system is used to remotely communicate with your device and send data to a central database. The patient management system is designed to tell your physician within 24 hours if alert conditions are detected. This can be done once a week or daily depending on the capabilities of your device. The devices also have the capability of detecting early signs of heart failure.
