Cholesterol Management Program

This program is designed to educate and manage cholesterol levels. Lowering cholesterol levels in important for all regardless of age, sex, race, or previous heart disease. Having an abnormal level of fat in the blood increases your risk for a heart attack or stroke; however, we believe one the best ways for you to lead a satisfying life is to raise your awareness and understanding of high blood cholesterol and to assist you to take part in managing your health.
By participating in one on one session with a registered nurse, you and your family will learn about dyslipidemia, including the effects of abnormal levels of the various types of cholesterol and triglycerides, medications, dietary recommendations, activity. The nurse will individualize an appropriate treatment regimen for your schedule and lifestyle. You will learn how to eat healthy, exercise safely, and decrease your risk of a heart attack or stroke. We will test your blood levels with a simple finger stick. You must be fasting for 9-12 hours prior to your visit. This means nothing to eat or drink, with the exception on taking your medication with a sip of water. Your results will be immediate and appropriate therapy will then be prescribed, if necessary.
The frequency of your visits will depend on your response to therapy and the educational material, and the severity of your medical condition. As a general rule, visits to the Cholesterol Management Program are once every four-six weeks until your blood levels are well controlled and the every six months. . We understand and respect the relationship you have developed with the physicians involved in your care. Participation in this program does not replace the visits you have scheduled with your primary care physician or cardiologist. We will continue to provide your physicians with regular updates regarding your progress through the course of the program. Clinical data will be forwarded to them at each visit to be reviewed on a regular basis. Collaboration with your Cardiologist occurs at each visit.

Heart Failure Program

This program is designed to manage you health by learning about heart failure and how to recognize early signs of trouble. Having heart failure may be difficult at times; however, we believe one of the best ways for you to lead a satisfying life is to take part in managing your heart failure.
By participating in one on one session with a registered nurse, you and your family will learn about heart failure and how to recognize early signs of problems. The nurse will review your current medications and work on developing the best combination and right schedule for you. You will learn how to eat healthy, exercise safely and manage day to day stress. The assistance we offer is providing you the educational tools you need to understand the disease process and what you can do to eliminate or minimize the symptoms associated with it. We want to improve your quality of life, keep you out of the hospital and provide for you individuals dedicated to assisting you with all aspects of your heart failure care.

Anticoagulation Management Program

This program is designed to educate and manage anticoagulants. Warfarin is an anticoagulant medicine that decreases the ability of blood to form clots. Coumadin and Jantovan are brand names for Warfarin. Blood clots may form in people who have artificial valves, irregular heartbeats (atrial fibrillation), or clotting disorders. Warfarin is different from other kinds of medicines. The effect of Warfarin on your body’s clotting system can change a lot from week to week even if you take the same amount of medicine. You will need to have blood tests regularly while you take this medication. The blood test is called PT/INR.
By participating in one on one session with our expert staff, you will learn what this medication can do and other healthy eating habits to work in harmony with this medication. We will test your blood levels with a simple finger stick. Your results will be immediate and the appropriate therapy will then be prescribed.
The frequency of your visits will depend on your response to therapy and the educational material. . The Anticoagulation Management Program does not replace the visits you have scheduled with your primary care physician or cardiologist. Amiodarone Management Program

Amiodarone can be very effective in treating your heart condition. Unfortunately, some unwanted side effects may occur as well. At Cardiac Solutions we monitor you in a unique way to make sure you are not experiencing any of these side effects.
By participating in a one- on- one session with a registered nurse, you will learn about the medication and how to recognize any potential problems. You will receive educational material on the explanation of the use of the medication. We will also arrange and schedule any necessary testing and review potential drug-to-drug interactions with you during your visit. The nurse will individualize an appropriate monitoring regimen for your schedule and lifestyle. Taking advantage of this program is an important step in maintaining the benefits from Amiodarone.
As a general rule, the frequencies of your visits are approximately four times a year as directed by the clinical staff. We understand and respect the relationship you have developed with the physicians involved in your care. The Amiodarone Management Program does not replace the visits you have scheduled with your primary care physician or cardiologist. We will continue to provide your physicians with regular updates regarding your progress through the course of the program. Pacemaker

Pacemaker and Defibrillator Clinic

Cardiac Solutions offers regularly scheduled clinics with a device representative to check your pacemaker or defibrillator on a regular scheduled basis as directed by your physician. In addition to your regular follow-ups your physician now has the capability of monitoring your defibrillator from home with a remote communicator. The patient management system is used to remotely communicate with your device and send data to a central database. The patient management system is designed to tell your physician within 24 hours if alert conditions are detected. This can be done once a week or daily depending on the capabilities of your defibrillator.

Enhanced External Counterpulsation (EECP®)

This procedure is a non-invasive, outpatient treatment that can relieve or eliminate angina. You may be a candidate for EECP® if you have chronic stable angina, are not receiving adequate relief from angina by taking nitrates, and are not a candidate for invasive procedures. This treatment appears to stimulate the opening of new, natural pathways around narrowed or blocked arteries.
By participating in a one on one session with our expect staff, and after EECP® treatment, you may find you can walk further, carry heavier packages and be more active and not longer have to restrict your social life and other activities. If you think you are a candidate for EECP® please ask you physician to evaluate you.
The frequency of your visits usually consists of one hour a day and takes between four - seven weeks to complete the treatments.
